Our RN’s provide the following health advocacy services:
Participate in doctor and hospital visits
Coordinate care with doctors, nurses, case managers, and families
Assist in a smooth transition to the next level of care whether it’s assisted living, rehabilitation, home care, long-term care, Hospice, etc.
Educate seniors and their families on health and healthcare options
Inform seniors of their rights and actively work to protect those rights
Connect seniors and their families to appropriate healthcare resources in the area
